Agreement
1. By entering the Prize Draw you are agreeing to these Prize Draw terms and conditions.
2. The Prize Draw is being run by Confidental Surgeries Ltd Company Number 06810661 (CSL)
Eligibility to enter & deadline
3. The Prize Draw is open to patients of Confidental Surgeries Ltd Company Number 06810661. All entrants must be over 18 years of age (CSL staff members are excluded from the draw).
4. In entering the Prize Draw, you confirm that you are eligible to do so and eligible to claim any prize you may win.
5. A maximum of one entry per individual is permitted.
How to enter
6. The Prize Draw will be made from entries received on or before midnight 31st August 2024. Entries received after that date will not be included in the Prize Draw.
7. The Prize Draw is free to enter via a tracking link included per patient in our August 2024 Newsletter. Entries can also be submitted via e-mail to info@confidentalsmiles.co.uk - you must be a patient and include your name and a contact phone number to be eligible.
8. CSL will not accept responsibility if contact details provided are incomplete or inaccurate.
The Prize
9. The prize will be a voucher for an Oral Health Pack, approximate value £25 (including delivery to the winner's chosen mainland UK address).
10. CSL's use of particular brands as prizes does not imply any affiliation with or endorsement of such brands.
11. The winner will be drawn at random from the number of completed entries by using https://www.randomdraws.com/uk/random-number-generator/
12. The prize is non-exchangeable, non-transferable and no cash alternatives will be offered.
13. CSL reserve the right to substitute prizes with another prize of equal or higher value if circumstances beyond our control make it necessary to do so.
14. The decision of CSL regarding any aspect of the prize draw is final and binding and no correspondence will be entered into about it.
Winner announcement
15. The winner will be notified via the email held in their patient record and/or phone number provided on the e-mail submission.
16. CSL will attempt to contact the winner by email and/or phone up to three times to arrange delivery of the Prize.
17. If the winner does not respond to the phone calls or emails notifying them of their win within 14 days of the third attempt, they will lose their right to the prize, and CSL reserves the right to choose and notify a new winner.
Receipt of the prize
18. Please allow 14 days for delivery of the prize. Alternative collection or delivery arrangements may be made through mutual agreement.
Data protection and publicity
19. By entering the Prize Draw, you consent to our Privacy Policy You also understand that your personal data may be used by CSL for the purposes of improving our services to patients, administering the Prize Draw, or any other purpose set out in our Privacy Policy.

21. The winner agrees to the release of their first name and Postcode District (ie GU47) to any other Prize Draw participants if requested via CSL.
22. An announcement of the winners first name (and a photograph if the winner provides this upon request) will be made via CSL on our website and social media.
23. All personal information can be used in accordance with CSL's Privacy Policy.
Limitation of Liability
24. CSL does not accept any liability for any damage, loss, injury or disappointment suffered by any entrants as a result of either participating in the Prize Draw or being selected for a prize, save that CSL does not exclude its liability for death or personal injury as a result of its own negligence.
25. CSL does not provide any form of practical or IT support for this prize. On receipt, all responsibilities relating to the Prize are that of the prize winner.
Amendments
26. CSL reserves the right to cancel the prize draw or amend these terms and conditions at anytime, without prior notice.
27. The Prize Draw and these terms and conditions will be governed by English law and any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.
